Balls Up!

I just read a story about a surgeon who was operating on a guy to remove a cyst from his testicle and removed the guys right testicle in error.

Despite this monumental fuck up he was allowed to continue in his post as a surgical locum and registrar.  This beggars the question, just how incompetent do you have to be in order to be struck off?
